The online survey is concurrently running across the college. It was initiated by the QAU with the support and assistance of ETC staff members. The Online Survey was first piloted in the ELC dated May 2012. The pilot test was successful. On the following semester, the online survey was partially implemented by different departments. And on 2nd semester AY 2012 – 2013, it was fully implemented. Three (3) types of approaches were used to administer the online survey in the HCT, and as follows:

|2||Restriction can be declared on the creation of the survey. The survey can be time bound but since this is an open survey available in the E-Learning Portal, the user can access the survey anytime they want. If survey is open for one week. All students enrolled in the course can access the survey. Teacher/s assigned to a course can also access the survey.||The survey is time bound; it is restricted and accessible only on the set schedule of the survey. No one can access the survey, because the IP address is confidential. Only the staff in-charge of the survey knows the IP address.||The survey is time bound. |4||Quota can be declared in the limesurvey. As the students and teachers access the E-Learning Portal, the link to the limesurvey will be available to them. There is no relational databases in the (2) two independent application. Assuming that the quota for the survey is 25 and the survey is available for one week. There is a possibilities that the students can access the survey several times. Since we declared a quota of 25, there is also a possibilities that as they exceed the quota, the other students cannot access the survey anymore. If quota is not declared, there is a chance that the number of respondents will be greater than the number of students enrolled in the course, referring to the actual class size.||Total respondents is relational to the class size.||Quota can be declare but not necessary since the survey is only available to a specific prospective respondent declared in the limesurvey. The ETC administer the online survey in the college with the proper guidance of the QAC. The instruction is coming directly from the QAC to ETC. The ETC communicates and coordinate with the concern academic and administrative departments on the respective survey tool which they are responsible.
There are two (2) types of survey running concurrently in the different departments for the Online Surveys and as follows: Opened survey – currently running in the ELC. In the Opened Survey, homepage for survey should be created like the common practice of ELC. The survey link is accessible in the survey homepage and available during on the set schedule of the survey. Closed Survey - the survey link is available through individual email account of the prospective respondents. Survey is restricted only to those email addresses declared in the limesurvey. Enough time is needed to administer the survey.
Along with the limesurvey, the Online Survey Report was created for accessing the Report. This system will allow the respective departments to easily access the survey report intended for them. Each report is in PDF format. The QAU has the full access to the survey report of all departments. The head of the department will be given the privilege to access the survey report for their department only.
The Online Survey Report used the methodology Rational Unified Process approach. It is a system development process framework that come across the needs of the user under time constraints. The Online Survey Report divided into phases. The 1st phase was successfully implemented during AY 2012 – 2013. The 2nd phase is about to implement this AY 2013 – 2014.
